Getting Started

 

  1. Consult with your child’s pediatrician and decide if OT services would benefit your child. If so,
    have them send us a referral.
  2. Check with your insurance company to inquire about OT benefits and if authorization is needed.
    Your insurance company will be able to let you know what to expect in terms of costs, the
    number of visits covered and more. *If you are with the Division of Developmental Disabilities
    and would like services, please contact your support coordinator to start the process.
  3. Give us a call so we can go over your needs and get you set up for an initial evaluation. If your
    child has been evaluated for OT, or received OT services, we will need to get copies of the
    records from his previous OT provider.
  4. Complete the necessary forms and bring them with you to your initial appointment. Links to
    these forms are below. *Additional information may be required based on your unique situation
    and your child’s age and/or reason for seeking OT services.
